Toernooivergoeding formulier
Door middel van dit formulier kan jij een deel (30%) van de inschrijfkosten terugkrijgen wanneer jij deelneemt aan een extern toernooi (buiten Tweeslag om georganiseerd). Je dient je declaratie binnen één maand in te leveren, anders zal deze niet vergoed worden! De declaratie zal zo snel mogelijk op de opgegeven rekening worden gestort.
Let op:
- Als je meerdere toernooien wilt declareren, vul dan voor ieder toernooi een apart formulier in.
- Zorg er voor dat er een foto van de inschrijving, bewijs van betaling én een foto waarbij jij kleding/stickers draagt van Tweeslag geüpload wordt (3 bestanden per aanvraag dus)
- Zorg ervoor dat de bon duidelijk en volledig zichtbaar is. Een foto is prima!
- Bewaar de bonnetjes totdat je het geld terug hebt. Als de bon niet duidelijk en/of volledig zichtbaar is, kan ik je vragen om een nieuwe foto.

By means of this form you can get a part (30%) of the registration costs back when you participate in an external tournament (organized outside Tweeslag). Please fill in the form within a month after the tournament, otherwise it will not be refunded! The invoice will be deposited into the specified account as soon as possible.
Pay attention to the following:
- If you want to declare more than one tournament, please fill in a separate form for every tournament
- Make sure there is a photo of the registration, proof of payment and a photo where you wear clothes/stickers from Tweeslag (3 files in total)
- Make sure the receipt is clear and fully visible. A photo is fine!
- Keep the receipts until you get the money back. If the receipt is not clear and/or fully visible, I can ask you for a new photo.
